Technology allows people with diabetes (PWD) to have access to health care professionals from other locations and from the comfort of home. In addition, PWD are using technologies for health monitoring including blood glucose, blood pressure, physical activity, meal tracking, sleep, and weight. Medicare now recognizes billing and reimbursement for remote monitoring. There is tremendous opportunity for diabetes care and education specialists (DCES) to use population health strategies and remote care to optimize health outcomes by providing the right care to the right patient at the right time. This webinar will demonstrate how the dietitian DCES can utilize technology, pattern management skills, remote monitoring, and coaching to enhance outcomes for the PWD.

Dawn has been a dietitian and CDCES for 16 years. She has experience working for the Cleveland Clinic Diabetes Center where she initiated a remote monitoring program for people with diabetes and obesity. Dawn is passionate about using nutrition, CGM technology, and digital healthcare to maximize the care and support for people with diabetes. Dawn is currently in private practice.
