This session will explore the current challenges and opportunities within the nutrition and exercise field to support people with kidney disease pursuing transplant. We will discuss the nutrition related barriers for kidney transplant and how dietitians can best support their patients in overcoming these barriers in preparation for transplant and how to maintain a healthy lifestyle post transplant.

In addition to nutrition specific barriers, we will discuss physical deconditioning and frailty and how dietitians can work with the interdisciplinary team to provide intervention. The audience will be provided with specific exercise recommendations, with focus on precautions that should be taken with this disease state, as well as a review of available community resources and financial support for exercise programs and gyms.
