Nutrition security or having equitable and stable availability, access, affordability and utilization of foods and beverages that promote well-being and prevent and treat disease is critical to health. As primary care practitioners and RDN's are working to support individuals in improving their health, it is paramount to identify their risk for food and nutrition insecurity and connect them with the resources they need to address those concerns.
This session will explore the tools available for identifying food and/or nutrition security in clinical and community settings as well as approaches to embedding a screening and referral system into practice. It will also discuss how to identify, connect with and support organizations meeting emergency food assistance and ongoing nutrition assistance needs and provide examples from working taking place connecting clinical and community resources. Finally, the session will discuss approaches for evaluating nutrition security interventions.
