The gut microbiome in people with cystic fibrosis (CF) is an important area of discussion and research. Caused by a defect in the cystic fibrosis transmembrane conductance regulator (CFTR) protein, CF promotes gastrointestinal (GI) inflammation and dysbiosis, which leads to long-term complications. Further, long-term antibiotic treatment, mucus production, diet, and intestinal pH can cause changes to the abundance, richness, and diversity of the gut microbiota, exacerbating GI symptoms. The use of probiotics to change the gut microbiome and ameliorate GI symptoms is not well known, but frequently seen in clinical practice. Since the introduction of CFTR modulators, many patients have seen improvements in nutritional status and GI symptoms. This raises the question of whether CFTR modulators are changing the gut microbiota and whether probiotic use has a place in standards of care. This article examines the current literature on probiotic use in people with CF and the impact of CFTR modulators on the gut.
