Learning Objectives

  • Session 1:
    • Formulate appropriate Total Parenteral Nutrition (TPN) macronutrient, micronutrient, and fluid requirements based on patient-specific clinical data. 
    • Analyze the clinical rationale for selecting specific TPN components to meet individual patient metabolic needs. 
    • Synthesize complex lipid profiles by strategically calculating fat emulsions to safely optimize caloric intake. 
  • Session 2:
    • Evaluate organizational and interdisciplinary barriers, including limited administrative support, inconsistent attendance and follow-through, communication gaps, and other factors that hinder effective nutrition risk meetings in post-acute and long-term care settings.
    • Analyze documentation and regulatory requirements for nutrition risk management and their impact on survey outcomes and quality measures. 
    • Develop a facility-specific action plan to improve nutrition risk meeting processes, interdisciplinary accountability, and clinical nutrition outcomes. 
  • Session 3:
    • Identify clinical conditions that require adjusted calculations for calories, protein and fluids for wound healing in residents with obesity or organ failure 
    • Describe key monitoring parameters to evaluate wound healing and nutrition effectiveness 
    • Determine appropriate use of wound-specific nutritional supplements to support healing while minimizing clinical risk 
  • Session 4:
    • Evaluate communication strategies for addressing nutrition related concerns with patients, families, and interdisciplinary team members while supporting informed, goal concordant decision making. 
    • Describe the physiological changes that occur during the dying process and thir implication for nutrition care, hydration, and appetite. 
    • Apply evidence based guidelines, ethical principles, and patient-centered goals to develop appropriate nutrition interventions for individuals receiving end of life care.

Amanda received her B.S. degree in Bachelor of Science in International Relations United States Naval Academy, Annapolis, MD and her M.S. in Family and Consumer Sciences University of Central Arkansas, Conway, AR. She served in the US Navy from 2009-2012. She was a consultant dietitian with Brummit and Associates from 2015 - 2018 when her husband was transferred to Germany. Amanda received the Navy and Marine Corp Achievement Medal. She has written several books: (2013). My Comfort in Affliction. (2012). Cooking When You Can't Chew. Currently Amanda is working with disabled pediatric patients. She lives in Marana AZ with her husband and 3 daughters.

Kim Fremont brings over 43 years of experience in healthcare and long-term care nutrition, with a background spanning clinical care, foodservice, and operations in both acute and post-acute settings. In her role as Co-founder and Senior Vice President of DiningRD she guides a consulting program supporting over 5,000 communities nationwide and collaborates with a team of more than 900 registered dietitians. Kim is passionate about equipping clinicians with practical, evidence-based strategies to navigate complex care challenges. She is a strong advocate for the role of nutrition and dining in long-term care, believing they are fundamental not only to clinical outcomes, but also to dignity, comfort, and quality of life—where excellent care and hospitality should always go hand in hand. Kim holds degrees in Medical Dietetics and master's degree in Instructional Technology and is an active member of the Academy of Nutrition and Dietetics and the DHCC practice group.

Rachell lives in the small timber community in central Idaho where she was born and raised, alongside her husband of 28 years. Together, they raised two sons who have made them proud grandparents. She has been with S&S Nutrition Network for 26 years, beginning her career as a consulting dietitian and advancing into a regional leadership role. Early on, she discovered her passion for post-acute and long- term care, where she found great fulfillment in supporting residents' nutritional well-being. Throughout her career, she has enjoyed providing leadership, guidance, and support to fellow consulting dietitians, helping them succeed while ensuring high-quality nutrition care for the individuals they serve.

Speaker - Food, Comfort, and Choice: Nutrition Through the End of Life

Shelby Rodriguez is a Registered Dietitian Nutritionist, Board Certified Specialist in Gerontological Nutrition, and Fellow of the Academy of Nutrition and Dietetics. She is currently the President of the Oregon Academy of Nutrition and Dietetics, and serves as a Subject Matter Expert for the Commission on Dietetic Registration. Shelby's professional experiences spans clinical nutrition, leadership, public health, and education. She recently served as the lead developer for the Academy's End of Life Microcertificate, helping create national educational resources to support dietitians in providing evidence based, compassionate care. Today, Shelby will share practical strategies for navigating nutrition care at the end of life, drawing on current evidence, clinical experience, and interactive case studies to help dietitians confidently support patients, families, and interdisciplinary care teams.
